To retrieve your previously issued credit code, please include the following information with your request:

• Full Name
• iPhone Serial Number
• iPhone Telephone Number
• Description of Issue

NOTE: The Serial Number is located on the back of your iPhone, on the iPhone’s product box, and on the receipt of purchase. It can also be found by turning your iPhone on, clicking Settings, then General, and then About.
